Preamble

This form is a template consent of a minor's legal representative for processing personal data in the KMS service under Arts. 9 and 18 of Law No. 179 of the Kyrgyz Republic of 31 July 2025 on Personal Data Protection.

Consent is given electronically through the parent's account in KMS by confirming the consent to data processing set out below. Fact, time, IP, and document version are recorded in the parent_consents table and can be provided on request.

Child (data subject)

• Full name: provided when registering the child
• Date of birth: provided when registering the child
• Kindergarten: filled automatically by the system

Data controllers

Under the joint controller model:

• Kindergarten — primary controller of educational and organisational data;
• Sole Proprietor Tursunakunova Nurzhan Surapovna (OKPO 33188711, reg. no. 004-2024-169-3867 of 2024-08-20) (KMS) — controller of the software platform and related processing.

Consent to processing of the child's personal data

Consent type: data_processing (required for enrollment).

I consent to processing the following data of my child:

• Full name;
• Date of birth, gender;
• Residential address;
• Birth certificate number and scan;
• Kindergarten group;
• Attendance and daily diary entries;
• Developmental assessments and pedagogical notes.

Purposes:

• Enrollment and registration of the child;
• Educational delivery;
• Communication with parents;
• Statutory reporting.

Operations: collection, recording, structuring, accumulation, storage, refinement, retrieval, use, blocking, deletion, destruction.

Term: while the child attends the kindergarten plus 3 years after discharge (to prove lawfulness in case of inspections).

Right to withdraw: at any time via KMS or at [email protected]. Withdrawing general consent makes continued attendance technically impossible.

General

Data subject rights (Arts. 10–13, 23):

• Access to processing information — within 14 days of request;
• Correction of inaccurate data — within 7 days;
• Deletion — within 7 days (where no other lawful basis remains);
• Complaint to the State Agency for Personal Data Protection — https://dpa.gov.kg.

Signing: consent is given upon confirming each block in the parent's KMS account. The signature is captured electronically with date, time, IP, and document version.

Prior consents: earlier consents (including paper ones) remain effective until revoked or updated in the service.
